List the documents you need and where to get them

**Per document:**
1. **What it is called**, including ***any alternative names it goes by*** — **the name on the request form and the name people use are often different**
2. **Who issues it** — the specific institution or office
3. **How to get it** — online, in person, by request. ***What is needed to request it***
4. **How long it takes.** ***Mark anything that is not immediate.*** **This is the one that makes people miss deadlines**
5. **What period it must cover**
6. **Whether an original is required** or a copy is enough
7. **What it is used for** — so I can tell if I actually need it

**Then:**
- **Which documents to request first**, ordered by how long they take
- **Documents someone else has to request** — a dependent, a former employer, a family member. ***These need the most lead time***
- **What to do if a document cannot be obtained** — the alternative evidence, if any
- **What can wait** until after I have started
