Prompt
**Open the output with this, written in the language you are answering in: "This is not medical advice and does not replace seeing a doctor."**
***Say it in the reader's language, not in English*** — **a disclaimer the reader skims past does nothing.**

Organize what I take into a list I can hand over. ***Organize only.***

**Per item:**
1. **The name as printed on the box or label** — ***including the exact spelling.*** **If I gave a nickname or a partial name, ask rather than guessing which product it is**
2. **The strength and form**, as printed
3. **How much and how often I actually take it** — ***what I really do, not what the label says***
4. **When I started**, and who prescribed it or whether I bought it myself
5. **What it is for**, ***in my words*** — **and mark where I am not sure**

**Include, because people leave these out:**
- ***Anything taken only sometimes*** — painkillers, sleep aids, antihistamines
- **Supplements, vitamins, herbal products, teas**
- **Anything applied rather than swallowed** — creams, drops, patches, inhalers
- **Anything prescribed and stopped**, with when and why
- **Anything I was told to take and do not**

**Then:**
- **What is incomplete** on this list and where to get it — the pharmacy, the prescription, the box
- **What to update it after**

⚠️ ***Do not comment on interactions, side effects, doses, or whether anything is appropriate. Do not say anything is safe or unsafe together. Do not suggest stopping, starting, or changing anything.*** **Those are for a pharmacist or doctor, who needs exactly this list to answer them.**
After pasting, fill in the fields at the bottom (What I take)

What this prompt does

Medication lists always omit the same things: occasional painkillers, supplements, creams and drops, and what was prescribed but not taken. This includes them and records what you actually do.
